The extent of loneliness was equally distributed between men and women, although women were more disadvantaged regarding living arrangements as well as physical and mental health. However, loneliness was stronger associated with adverse mental health conditions in men. These findings should be considered when developing intervention strategies to reduce loneliness.
Psychoanalytic therapy shows significantly longer-lasting effects compared to cognitive-behaviour therapy three years after termination of treatment, which is discussed as a dose-effect.
Idiopathic chronic pain conditions with a mismatch between anatomical abnormalities and symptoms can be categorized as somatoform pain disorder according to the DSM-IV criteria. A dysfunction of pain processing circuits has been suggested as one underlying pathophysiological factor. There is accumulating evidence for a crucial role of affect regulating brain structures such as the medial frontal cortex in this context. We investigated the cerebral processing of noxious heat stimuli as objective marker for pain sensation in 12 right handed women with somatoform pain disorder fulfilling DSM-IV criteria and 13 age-matched healthy volunteers using functional MRI. The average ratings for experimentally induced pain were not significantly different between controls and patients concerning pain intensity and pain unpleasantness. Comparing patients with controls a pain related hypoactive state of the ventromedial prefrontal/orbitofrontal cortex (BA 10/11) and a hyperactive state of the parahippocampal gyrus, amygdala and anterior insula were found in the patient group. Our findings of an altered cerebral processing of experimentally induced pain in patients with somatoform pain disorder support the hypothesis of dysfunctional pain processing, especially in affect regulating regions.
In diesem Artikel wird eine deutschsprachige Kurzskala des Inventars der Persönlichkeitsorganisation (IPO-16) vorgestellt und in drei klinischen Stichproben mit insgesamt 1300 Personen validiert. Das IPO ist ein international verbreitetes Selbsteinschätzungsinstrument, das in mehreren Versionen vorliegt und zur Erfassung von struktureller Beeinträchtigung in den Bereichen Identität, Abwehr und Realitätsprüfung eingesetzt wird. Die Kurzskala basiert auf 16 versionsübergreifenden IPO-Items und misst den Schweregrad der strukturellen Beeinträchtigung. In den Studien konnte gezeigt werden, dass das IPO-16 (1) intern konsistent ist, (2) eine modellkonforme Faktorenstruktur hat, (3) eine sehr hohe Überlappung zur IPO-Vollversion aufweist, (4) in der Selbsteinschätzung über konvergente und diskriminante Validität verfügt, (5) den Schweregrad der Persönlichkeitsstörung aus Expertensicht unabhängig vom aktuellen Leidensdruck vorhersagt und (6) eine diagnostische Effizienz erreicht, die mit anderen Screening-Instrumenten im Bereich der Persönlichkeitsstörungsdiagnostik vergleichbar ist. Mit dem IPO-16 liegt für den deutschsprachigen Raum erstmals ein ökonomisches und gut validiertes Selbsteinschätzungsinstrument zur Erfassung von struktureller Beeinträchtigung vor.
The aim of this study was to investigate the effectiveness of long-term psychoanalytic and psychodynamic psychotherapies. In a prospective, randomized outcome study, psychoanalytic (mean duration: 39 months, mean dose: 234 sessions) and psychodynamic (mean duration: 34 months, mean dose: 88 sessions) therapy were compared at post-treatment and at one-, two-, and three-year follow-up in the treatment of patients with a primary diagnosis of unipolar depression. All treatments were carried out by experienced psychotherapists. Primary outcome measures were the Beck Depression Inventory and the Scales of Psychological Capacities, and secondary outcome measures were the Global Severity Index of the Symptom Checklist 90-R, the Inventory of Interpersonal Problems, the Social Support Questionnaire, and the INTREX Introject Questionnaire. Interviewers at pre- and post-treatment and at one-year follow-up were blinded; at two- and three-year follow-up, all self-report instruments were mailed to the patients. Analyses of covariance, effect sizes, and clinical significances were calculated to contrast the groups. We found significant outcome differences between treatments in terms of depressive and global psychiatric symptoms, personality functioning, and social relations at three-year follow-up, with psychoanalytic therapy being more effective. No outcome differences were found in terms of interpersonal problems. We concluded that psychoanalytic therapy associated with its higher treatment dose shows longer-lasting effects.
Psychoanalytic therapy is an effective treatment for major depression, especially in the long run. The differential effectiveness of psychoanalytic therapy cannot be fully explained by its higher dose. Distinctive features of psychoanalytic technique (e.g., focusing on patients' dreams, fantasies, sexual experiences or childhood memories) may play an important role in establishing sustained therapeutic change.
A review of the literature on migraine and personality yielded strong evidence for secondary neuroticism and increased sensitivity to stress in patients with migraine. This study focused on the identification of specific stressful situations and coping strategies in such patients. We conducted a psychodiagnostic study of 30 migraine patients in accordance with the criteria of the Headache Classification Committee of the International Headache Society and 30 healthy control subjects matched for age, sex, and social status. All participants completed the Minnesota Multiphasic Personality Inventory (MMPI) and special questionnaires on stressful situations and coping strategies. The migraine patients had higher neuroticism and introversion scores on the MMPI than the healthy subjects. There was a positive correlation between the neuroticism score and headache duration (number of hours per week). The patients used coping strategies characterized by the development of physical symptoms, social isolation, and preoccupation with stress. They rated themselves as less calm, less capable of relaxing, and more irritable than did the healthy controls subjects, and they responded more often with internal tension, especially in work and other achievement situations. Questionnaires that measure constructs dealing with stress yield information that is more relevant for the treatment of migraines than do global personality tests.
