The coronavirus disease 2019 (COVID-19) pandemic is having a profound effect on all aspects of society, including mental health and physical health. We explore the psychological, social, and neuroscientific effects of COVID-19 and set out the immediate priorities and longer-term strategies for mental health science research. These priorities were informed by surveys of the public and an expert panel convened by the UK Academy of Medical Sciences and the mental health research charity, MQ: Transforming Mental Health, in the first weeks of the pandemic in the UK in March, 2020. We urge UK research funding agencies to work with researchers, people with lived experience, and others to establish a high level coordination group to ensure that these research priorities are addressed, and to allow new ones to be identified over time. The need to maintain high-quality research standards is imperative. International collaboration and a global perspective will be beneficial. An immediate priority is collecting high-quality data on the mental health effects of the COVID-19 pandemic across the whole population and vulnerable groups, and on brain function, cognition, and mental health of patients with COVID-19. There is an urgent need for research to address how mental health consequences for vulnerable groups can be mitigated under pandemic conditions, and on the impact of repeated media consumption and health messaging around COVID-19. Discovery, evaluation, and refinement of mechanistically driven interventions to address the psychological, social, and neuroscientific aspects of the pandemic are required. Rising to this challenge will require integration across disciplines and sectors, and should be done together with people with lived experience. New funding will be required to meet these priorities, and it can be efficiently leveraged by the UK's world-leading infrastructure. This Position Paper provides a strategy that may be both adapted for, and integrated with, research efforts in other countries.
Policy-makers are considering large-scale programs aimed at self-control to improve citizens’ health and wealth and reduce crime. Experimental and economic studies suggest such programs could reap benefits. Yet, is self-control important for the health, wealth, and public safety of the population? Following a cohort of 1,000 children from birth to the age of 32 y, we show that childhood self-control predicts physical health, substance dependence, personal finances, and criminal offending outcomes, following a gradient of self-control. Effects of children's self-control could be disentangled from their intelligence and social class as well as from mistakes they made as adolescents. In another cohort of 500 sibling-pairs, the sibling with lower self-control had poorer outcomes, despite shared family background. Interventions addressing self-control might reduce a panoply of societal costs, save taxpayers money, and promote prosperity.
The strongest evidence that cannabis use may be a risk factor for later psychosis comes from a Swedish cohort study which found that heavy cannabis use at age 18 increased the risk of later schizophrenia sixfold.1 2 This study could not establish whether adolescent cannabis use was a consequence of pre-existing psychotic symptoms rather than a cause. We present the first prospective longitudinal study of adolescent cannabis use as a risk factor for adult schizophreniform disorder, taking into account childhood psychotic symptoms 3 antedating cannabis use. Methods and resultsThe Dunedin multidisciplinary health and development study (a study of a general population birth cohort of 1037 individuals born in Dunedin, New Zealand, in 1972-3) 4 has a 96% follow up rate at age 26. It obtained information on psychotic symptoms at age 11 and drug use at ages 15 and 18 from self reports and assessed psychiatric symptoms at age 26 with a standardised interview schedule to obtain DSM-IV (diagnostic and statistical manual of mental disorders, 4th edition) diagnoses. We analysed data from a representative group of 759 (74%) living study members who had complete data on adult psychiatric outcomes, adolescent use of illicit substances, and childhood psychotic symptoms.We divided the sample into three groups based on cannabis use at ages 15 and 18. The 494 controls (65.1% of the sample) had reported using cannabis "never" or "once or twice" at both ages; cannabis users by age 18 (236; 31.1%) first reported using cannabis "three times or more" at age 18; and cannabis users by age 15 (29; 3.8%) had reported using cannabis "three times or more" at age 15 (all of whom continued to use cannabis at age 18).Psychiatric outcomes at age 26 were symptoms of schizophrenia and depression and diagnoses of schizophreniform disorder and depression.Multiple linear regression analyses showed that cannabis users by age 15 and by age 18 had more schizophrenia symptoms than controls at age 26 (table). These results remained significant after psychotic symptoms at age 11 were controlled for. The effect was stronger with earlier use.Logistic regression analyses showed that people who used cannabis by age 15 were four times as likely to have a diagnosis of schizophreniform disorder at age 26 than controls. After psychotic symptoms at age 11 were controlled for, the risk for adult schizophreniform disorder remained higher among those who used cannabis at age 15; however, this risk was reduced by 31% and was no longer significant.
Bullying victimization is a topic of concern for youths, parents, school staff and mental health practitioners. Children and adolescents who are victimized by bullies show signs of distress and adjustment problems. However, it is not clear whether bullying is the source of these difficulties. This paper reviews empirical evidence to determine whether bullying victimization is a significant risk factor for psychopathology and should be the target of intervention and prevention strategies. Research indicates that being the victim of bullying (1) is not a random event and can be predicted by individual characteristics and family factors; (2) can be stable across ages; (3) is associated with severe symptoms of mental health problems, including self-harm, violent behaviour and psychotic symptoms; (4) has long-lasting effects that can persist until late adolescence; and (5) contributes independently to children's mental health problems. This body of evidence suggests that efforts aimed at reducing bullying victimization in childhood and adolescence should be strongly supported. In addition, research on explanatory mechanisms involved in the development of mental health problems in bullied youths is needed.
Children who are bullied-and especially those who are frequently bullied-continue to be at risk for a wide range of poor social, health, and economic outcomes nearly four decades after exposure. Interventions need to reduce bullying exposure in childhood and minimize long-term effects on victims' well-being; such interventions should cast light on causal processes.
In the age group committing most violent incidents, individuals with mental disorders account for a considerable amount of violence in the community. Different mental disorders are linked to violence via different core explanations, suggesting multiple-targeted prevention strategies.
Hallucinations and delusions, the classic symptoms of psychosis, have recently been documented to occur at a much higher prevalence in the general population than clinically diagnosed psychotic disorder.1 A meta-analysis of prevalence studies of psychotic symptoms in young people demonstrated a median prevalence of 17% in children aged 9-12 years and 7.5% in adolescents aged 13-18 years.2 As the term suggests, psychotic symptoms have typically been considered to relate to psychotic disorder. Indeed, research has shown that members of the general population who report psychotic symptoms share a wide range of risk factors with people with schizophrenia (see Kelleher & Cannon for review), [3][4][5][6][7][8][9] and young people who report psychotic symptoms have been found to be at increased risk of psychotic disorder in adulthood.10,11 Individuals who report psychotic symptoms are also more likely to report non-psychotic psychopathological symptoms, especially symptoms of depression; [12][13][14][15][16] Yung et al, for example, reported that individuals who had a diagnosed depressive disorder endorsed an increased number of psychotic symptoms on the Community Assessment of Psychic Experiences questionnaire compared with controls.17 Bartels-Velthuis et al found that young adolescents who disclosed psychotic symptoms were approximately 3-5 times more likely to score in the clinical psychopathology range on the parent-completed Child Behavior Checklist. Community-based studies to date, however, have relied mainly upon questionnaires to assess psychotic symptoms and have involved limited data on non-psychotic psychopathology. In addition, although research suggests that psychotic symptoms are more common in younger than in older children, 2 there is a lack of information on whether there are differences in the clinical significance of psychotic symptoms across different stages of adolescence. In an attempt to improve our understanding of the clinical significance of psychotic symptoms in the general population we examined data from four population studies, comprising two large population surveys and two in-depth clinical interview studies of psychotic symptoms. The aims of this work were to investigate whether psychotic symptoms predicted nonpsychotic clinical diagnoses, and if so, which disorders; to investigate whether psychotic symptoms predicted more clinically severe disorder in terms of comorbid psychopathology (i.e. having more than one diagnosis); and to investigate whether the significance of psychotic symptoms varied as a function of age.
